Marram Health Center is seeking a Quality Improvement Nurse to recommend and oversee implementation of all medical policies and procedures for a Federally Qualified Health Center, ensuring the safety and well-being of the clients and utilizing these programs.

Marram Health Center is a Federally Qualified Health Center (FQHC) with two offices in Gary and Hebron. Marram Health Center is affiliated with Porter-Starke Services, Inc., a Community Mental Health Center (CMHC) based in Valparaiso that serves northwest Indiana with additional offices in Portage, Knox, and LaPorte.

  • Provide oversight for LPNs, Medical Assistant Supervisors and Medical Assistants in an effort to ensure that patient care parameters are met.
  • Manage the Admit-Discharge-Transfer (ADT) process for coordination of care
  • Manages the infection control program
  • Provides education to staff regarding infection control and other clinical issues
  • Manage care coordination and referral process
  • Participate in design/determination and implementation of policy and procedures, quality improvement projects and workflows related to patient care
  • Participate in and provide reports to Quality Improvement Committee and projects
  • Demonstrates the knowledge and skills necessary in providing care based on physical, psychosocial, educational, safety and related criteria appropriate to the age of the patients served.
  • Coordinates patient care with other health care team members, departments, agencies, hospitals, managed care, and insurance companies as indicated.
  • Organize patient care based on findings of initial assessment.
  • Perform nursing services as necessary
  • Execute general laboratory services and procedures, such as triage, phlebotomy, immunizations, other injections, submission of specimens, and tracking of results. 
  • Assess, evaluate and plan care for patients for routine treatment, acute care and chronic disease maintenance.
  • Provides education to patients about illness and illness management per medical practitioner direction
  • Monitor patient status and report abnormal findings or changes in condition.
  • Assist provider with on-site procedures.
  • Maintains patient’s medical record and charts the following: nursing observation; vital signs; reactions to test, i.e., PPD results, in-house testing, and other pertinent information.
  • Attends and participates in in-service education.
  • Possess strong computer skills including proficiency in Excel and Word
  • Demonstrate excellent interpersonal and communication skills.
  • Possess strong organizational skills
  • Proficient in correct English usage, spelling and grammar.
  • Proficient in oral and written communication procedures.
  • Perform other duties as assigned. 


  • Licensed as a Registered Nurse in the State of Indiana.
  • At least two years’ experience in clinical setting preferred.
  • Previous experience working in a Federally Qualified Health Center is preferred.
