Porter-Starke Services is seeking a LifeHouse Coordinator who oversees and coordinates all aspects of transitional supervised group living (SGL) program LIFE House and Davies Home serving adults diagnosed with serious mental illness (SMI).

This position is full-time and will work in Valparaiso, Indiana.

Porter-Starke Services, Inc. is a Community Mental Health Center (CMHC) based in Valparaiso and serves northwest Indiana with additional offices in Portage, Knox, and LaPorte.  Porter-Starke also operates Marram Health Center, a Federally Qualified Health Center (FQHC) with two offices in Gary and Hebron.

  • Supervises the day-to-day operations of LIFE House; assigns, supervises and directs staff to provide for 24-hour coverage and to ensure the safety of residents and staff
  • Evaluates resident referrals and consults with treatment team regarding placement, treatment and discharge of residents
  • Provides ongoing training to staff re: client engagement, treatment interventions, de-escalation techniques, crisis intervention skills, creating a positive environment, etc.
  • Works with treatment team to coordinate services of other departments directly involved in providing services to the residents
  • Facilitate increase in daily living skills of residents with goal to re-integrate to the community and a lower level of care among housing programs (e.g., LIFE House to Davies Home to Independent Living)
  • Prepares reports and documentation as required by regulatory bodies and organization
  • Demonstrates proficiency in electronic medical record documentation and assists staff in developing quality documentation skills
  • Monitors and works to improve the productivity of staff and maintains awareness of financial position of housing programs
  • Demonstrates a ‘can do’, positive attitude and sense of humor even during difficult situations
  • Demonstrates effective problem-solving skills with a commitment to innovative solutions
  • Enjoys providing great person-centered customer service and finds great satisfaction in working with a diverse population of people needing special housing supports

  • Participate in weekly staffing with psychiatrist and direct housing team meetings
  • Model openness to creative, innovative and effective ways to work with our clients
  • Performs other duties as assigned


Must demonstrate skills in the following:

  • Treatment of seriously and persistently mentally ill adults, signs and symptoms of psychiatric disorders, and appropriate interventions to provide for stabilization of escalating mental health symptoms
  • Strong ability to engage and work with individuals with SMI to promote individual recovery and self-determination
  • Awareness of guidelines and compliance with safety regulations related to a multi-person residential facility
  • Understanding of and compliance with all state and accrediting body regulations
  • Provides leadership in relation to day-to-day operations, treatment interventions, and documentation
  • Utilization of team-oriented approach and effective organizational abilities
  •  Strong interpersonal and assessment skills
  • Proficient use of the electronic medical record
  • Excellent written and verbal communication skills
  • Working knowledge of evidence-based practices, including MI, I-IMR, WRAP, etc.



  • Master’s degree in psychology, social work or related human services field required; license preferred
  • Bachelor’s Degree in psychology, social work or related human services field required
  • Experience and knowledge in chronic disease management, physical healthcare maintenance and service delivery, and nutrition preferred
  • At least three years of experience working with adults with SMI required
