Porter-Starke Services, Inc. is seeking a Health Mentor (Certified Personal Trainer) that will assist InSHAPE participants to develop and implement their personal wellness program. The mentor will care for adults diagnosed with a serious mental illness and clients from this population that most commonly have a co-occurring chronic physical health condition.

This position is full-time and will work at our Valparaiso location.

Porter-Starke Services, Inc. is a Community Mental Health Center (CMHC) based in Valparaiso and serves northwest Indiana with additional offices in Portage, Knox, and LaPorte.  Porter-Starke also operates Marram Health Center, a Federally Qualified Health Center (FQHC) with two offices in Gary and Hebron.

  • Provides community based support services to ensure implementation of clients’ health goals.

    • Serves as a resource to staff on client wellness activities and integrated care and education
    • Manages a caseload of at least 15-20 clients in our wellness programming to support and implement individual wellness plans.
    • Supports clients with learning and implementing wellness based interventions for mental illness management including: coping strategies, exercise, mindfulness, cooking skills, nutrition education, tobacco cessation, and assisting clients with accessing community resources to improve health.
    • Facilitates evidence based group curriculum's to demonstrate/teach cooking skills, nutrition, physical activity, tobacco cessation, etc.
    • Provides frequent re-evaluation of wellness goals and progress.
    • Actively models and participates in interventions with clients in the community setting.
    • Encourages and engages with clients via Motivational Interviewing strategies to support lifestyle changes for improved mental and physical health outcomes.
    • Administers fitness assessments as needed and record/reports data for outcome measuring.
    • Completes all necessary documentation for services provided to clients in accordance with Treatment Plan goals.
    • Works collaboratively with case managers, therapists, psychiatrists, and community partners to increase client engagement in treatment and progress towards recovery.
    • Performs other duties as assigned. 


  • Nationally accredited personal training certification (ACSM, ACE, NASM, NETA)
  • Bachelor Degree in Human Services field required; bachelor's degree in Health Science, Social Work, Rehabilitation, Psychology, Health Promotion, or related field preferred
  • Experience and/or training in motivational interviewing and motivational strategy building
  • Physically active, readily identifies and believes in the holistic benefits of good health
  • A commitment to decreased health disparity and social stigma for individuals with mental illness



  • Must possess a valid driver’s license.
  • Must be CPR certified (if not, training is provided)
  • Must be CPI certified (if not, training is provided)
